How did we do?
Your experience matters to us.
When Park Memorial is asked to provide support to your family — whether through preplanning and end-of-life care, funeral arrangements, document service and aftercare, or grief support — your feedback matters to us, so that we can continually strive to provide compassionate, thoughtful care to every family we serve. Your feedback – positive or constructive – helps us to do that.
If there is anything we could have done better or differently, we sincerely hope you will let us know directly so we may address it with care and sensitivity. Calling or sending us an email allows us to listen and learn, while giving us an opportunity to make things right as best we can. Your family deserves personal and attentive care. Not only are we committed to providing it; we are always committed to improving on it.
We understand that everyone grieves differently, and some families prefer to keep everything private. There is absolutely no pressure to share publicly. If you do feel comfortable sharing publicly, your kind words about your experience may help other families who are seeking care and guidance because when a family is walking through loss, they’re often making decisions in grief and fog, or uncertainty. You can be as descriptive or as brief as you’d like — a couple of paragraphs, a couple of sentences, or even just a few words.
Choose how you’d like to share:
Option 1:
Share feedback privately.
If you would like to share your thoughts with us privately, please do via the form below:

Option 2:
Leave a public review.
Families often look for reassurance, kindness and trust when choosing a funeral home.
Hearing from someone who has been there can help them feel less alone and more comfortable during a difficult time.
Not sure what to write?
If you’re unsure where to start, here are a few examples:
"What meant the most to you?"
"Was there a moment or detail that brought comfort?"
"Did someone on our team make a difference?"
"What would you want another family to know?"
Team Recognition
If a staff member stood out and positively impacted your experience, we would be grateful to hear that, whether it’s someone who answered your questions on the phone, your funeral director, or your vehicle driver.
We make a point of sharing your kind words directly with the team member(s) you mention.
Thank you for placing your trust in Park Memorial.
Thank you for your feedback; if there is anything you feel we could have done better — even something small — I ask that you share that with us. We will listen with care, because your experience matters, and we are always learning. If you need support in the days ahead, we will continue to be here for you.
Sincerely,

Kirstie Smolyk
President
